CHECKLIST FOR APPLYING FOR

PASSPORT.

PROCEDURE TO APPLY FOR PAKISTANI PASSPORT IS AVAILABLE AT WWW.PAKISTAN.ORG.AU  AND AT CONSULATE

While applying for a new passport the following documents are required to be attached along with the last passport in original:

• Duly filled in Form 'A' (for a person over 12 years of age) or Form 'B' (for children under 12 years of age. Both parents consent duly signed by J.P. may be asked for). Three (3) recent 2x2 size photographs in matte finish with name of the applicant on the back (not less than 6 months old)
• Two recent 2x2 size photographs in matte finish of each child under 12 years if required to be endorsed in parent's passport. Both parents consent duly signed by J.P. may be asked for.
• Original as well as copy of National Identity Card for Overseas Pakistanis (NICOP)/(CNIC), Computerized Registration Certificate (CRC) in case of children under 18 years issued by NADRA duly attested by JP.  In case you are applying to the Consulate General of Pakistan, Sydney, kindly make payment in favor of "Consulate General of Pakistan, Sydney". Payments to Consulate General of Pakistan, Sydney may be made by Money Order, Certified bank Check/Bank draft. Payments can also be made through Citilink Finance.
• Pre-paid self-addressed envelope, registered post or courier satchel, unless picking up personally. {High Commission/Consulate accepts no responsibility for any documents so dispatched by it .)

KEY: CNIC - Computerized National Identity Card, NICOP - National Identity Card for Overseas Pakistanis, POC Pakistan Origin Card, CRC - Computerized Registration Certificate & JP - Justice of Peace.

Note: Govt. officials must submit departmental NOC ( No Objection Certificate) for any passport facility.

LOST PASSPORT

A new passport in lieu of a lost passport is issued after verification by the issuing authority which usually takes six to eight weeks or more. Application for issuance of a new passport in lieu of a lost passport in addition to the above requirements also must be accompanied by the following:

·         Original Police Report

• Duly filled in report of Lost Passport on the prescribed form (4 pictures to be attached)
• Dully filled in Form-A   (Three 2x2 size photographs in matte finish to be attached)

Note: 7 pictures in total required for a Lost passport application.

FEE TABLE 

At third time loss no passport but a single journey travel document will be issued.
